How to Nominate

Nomination Process

Any staff at Schwartz Center member organizations can nominate a program or initiative. 

Please review our criteria before you start. You will need to complete an online nomination form with the following information:

  • Your name and contact information.
  • A nomination form that will ask you to provide a description of the program, explain what inspired your organization to create this program, what needs it was designed to meet, how it has made an impact in your organization or system, and who the program serves.
  • It is optional but encouraged to submit additional supporting materials (e.g., letters from staff, marketing collateral). PDFs are preferred.

You will have an opportunity to save your work and return to it at a later time before you submit. However, we can’t accept additional or changed materials once you have submitted. 

Nominations are now closed for 2024.
